Integrated Services Project
PROGRAM
SUMMARY: Integrated
Services Project is a project
designed to provide nontraditional and creative services to adults with mental
illness.Services are voluntary
and focus on helping meet client goals and to foster independence.
ISP staff members are organized in teams and provide direct
assistance, help coordinate or purchase services, and help clients to access
whatever mental health services they need.
Staff members help clients to enroll in traditional services and help
them learn to access community resources and to develop community supports.
A client center is an integral part of the project; it is designed to
assist the clients in staying connected socially, and orient them to new
entertainment and activity opportunities.
Clients
remain in the program for an extended period. This is planned due to many having an extensive history of psychiatric
instability, homelessness, or substance abuse.
They remain within the program for an extended period to give them an
extended period of support and stability and to ultimately foster
independence.Clients may request
un-enrollment from the program at any time.
If the client requests un-enrollment, staff make every attempt to
ensure needed services are in place to support the clients in the transition
and in their independence.

FUNDING SOURCE: Polk County Health Services, Flat
Monthly Rate Per Client
HOW TO
MAKE A REFERRAL: Referrals are
received as assignments from Polk County Health Services.
All requests should be directed to Utilization Management Coordinator

PROGRAM GOALS: To be responsive to effectively assisting clients in meeting their goals and needs. To support clients in achieving stability in outcome areas identified by Polk County Health Services.To strive to continue to develop and provide the best Integration services possible.
