Clinical Outpatient Program PROGRAM SUMMARY: Clinical outpatient services are provided at 1301 Center Street Des Moines and at 301 N. Buxton in Indianola. Services include individual, group, couple and family therapy and are provided by mental health professionals. Clinical social workers, a clinical psychologist and mental health counselors make up the well-qualified therapy team. A bilingual (Spanish/English) therapist is available. Services include assessments and counseling for a wide range of issues/diagnoses including marital counseling, depression, anxiety, schizophrenia, bipolar disorder, anger management, co-occurring mental health and substance abuse.Present therapy groups are: men and women’s Dual Diagnosis groups, Recovery from Depression, Healthy Relationships, Anger Management, Emotional Intensity Management, and Panic Disorder/Anxiety. At times, the clinic offer a Survivors of Sexual Abuse group and is open to offering other groups as needed and requested. Psychological evaluations and IQ testing are also available. An essential component of the clinical program is the psychiatric services. An experienced team of two psychiatrists, a nurse practitioner and two mental health nurses offer psychiatric assessments and medication management. Several team members provide consultation and education to other providers as well as to community organizations in Polk and Warren counties. Team members are available to speak with businesses and community groups. The clinic is open five days a week, including one evening. Psychiatric services are provided five days a week, also including one evening. ELIGIBILITY CRITERIA: The clinical outpatient program primarily serves persons 17 and older residing in Polk and Warren counties. Eligibility is based on funding for services, as it is a fee for service program; however, the clinic is able to secure funding for most persons who apply for services. Generally, persons do need to have a DSM-IV diagnosis to continue services, unless the person is able to personally pay full fee.
PRIMARY FUNDING SOURCE: As a fee for
service provider, the largest source of revenue is from Polk and Warren

Clinic staff solicit feedback from consumers, other providers and community members. A portion of the consumers will continue in the program for years or lifetime, due to the nature of the illness. Some of the consumers may come only for medication management, after completing therapy. Supported Community Living: SCL is a mental health treatment program designed for persons with a serious and persistent mental illness. The overarching goal of the program is to increase the consumer’s level of functioning and to assist consumers in gaining or regaining control over their own lives. The program components include: outreach; meeting basic human needs; rehabilitation; crisis intervention; assistance with housing and living arrangements; coordination and development of natural supports; protection and advocacy; and service coordination. Supported community living is an Eyerly-Ball community Mental Health Services program housed at Golden Circle Behavioral Health Intensive Psychiatric Rehabilitation
