Supported Community Living

Service Summary:  Supported Community Living (SCL) services are designed to support individuals as they live and work in the community.  These services address mental and functional disabilities that negatively affect integration and stability in the community. 

 Eligibility Criteria:

• The individual is at least 18 years old and has a primary diagnosis of a mental illness.

• The individual can benefit from some form of community living support.

 Program Goals:  Successful completion of the program consists of the consumer having met the goals and learned the skills that were established for increased independence.

Release of Information

The SCL program would like to express our appreciation to Prairie Meadows for granting us $10,000 in 2005.This money will be utilized to provide a variety of activities for our clients who otherwise have few supports and do not get out in the community often.
