Service
Summary:Targeted Case Management (TCM) is an individually centered, family and community focused process
through which we assist in building a foundation for future growth by
identifying, securing, and sustaining resources necessary for our consumers
to live in a normally interdependent way within the community.
Eligibility
Criteria:
The individual is receiving or is eligible
to receive Medicaid benefits; and
The individual is at least 18 years old
and has a primary diagnosis of mental retardation, developmental
disability, chronic mental illness, or is receiving services through the Home
and Community Based Waiver
The individual is under the age of
18, and is receiving services through the Home and Community Based Services (HCBS) Waiver/Mental Retardation.
There must be a need for the service
(services that need to be accessed, coordinated, monitored and funding
procured).
Targeted Case Management is a
voluntary program, so the client must also want the service and agree to
participate.
