Targeted Case Management

Service Summary:  Targeted Case Management (TCM) is an individually centered, family and community focused process through which we assist in building a foundation for future growth by identifying, securing, and sustaining resources necessary for our consumers to live in a normally interdependent way within the community.

 Eligibility Criteria:

  • The individual is receiving or is eligible to receive Medicaid benefits; and

  • The individual is at least 18 years old and has a primary diagnosis of mental retardation, developmental disability, chronic mental illness, or is receiving services through the Home and Community Based Waiver

  • The individual is under the age of 18, and is receiving services through the Home and Community Based Services (HCBS) Waiver/Mental Retardation.

  • There must be a need for the service (services that need to be accessed,  coordinated, monitored and funding procured).

  • Targeted Case Management is a voluntary program, so the client must also want the service and agree to participate.

Program Goals:  To provide a high-quality service to our consumers and to meet or exceed Polk County Health Services outcome targets.
