PROGRAM SUMMARY:

Westminster House is a residential care facility with emphasis on Rehabilitation Services. Skills training provided includes but is not limited to; cooking, budgeting, anger management, hygiene, cleaning, social skills, learning community resources, and medication management.

Westminster House has two facilities. We provide 24-hour supervision to 15 residents in each facility.

PROGRAM GOALS:

To provide quality care to our consumers.

To provide the least restrictive living environment.

To assist the consumer in reaching their goals so they may live a more satisfying life.

PROGRAM CRITERIA:

Consent to admission and be eighteen (18) years of age or older.

Consumer must have a case plan upon admission. This case plan will include Adult Rehab Options Skills.

Applications will be considered on a first received, first reviewed basis. (Exception: refer to preference #10 below).

All applicants will be screened and selected in full accordance with all federal, state, local housing and equal opportunity laws, and preference requirements.

All applicants will be considered for admission based on Westminster House policy and procedure for non-discrimination in regard to race, creed, color, religion, age, gender, ethnic/national origin and/or handicapping condition.

Perspective consumers must have a verified legal settlement in a county or must be a resident of Iowa. Approval for funding must be completed before the consumer can be admitted.

Perspective consumer must meet "very low income" limits as defined by the Department of HUD.

Consumers must have primary psychiatric diagnosis and a demonstrated need for this level of care.

Preference will be given to applicants with one of more of the following:

   a. Program eligible consumers paying more than 50%, of their gross monthly income for rent and utilities.

   b. Program eligible consumers who have been involuntarily displaced due to fires, disasters, government actions, or actions by private owners that the consumer cold not control or prevent does not include a rent increase or eviction with cause.

   c. Consumers certified to be living in substandard housing as defined by HUD regulations.

Affirmative Marketing guidelines and objectives will be utilized in the marketing program and selection of residents.

Consumer will have no serious medical conditions which require daily nursing care.

Consumer has not required hospitalizations for physical aggression or suicide threats or gestures in the last three months. Exceptions will be made when there is firm evidence that placement in the program should eliminate cause(s) for those behaviors.

Consumer will have a history of one or more placements or demonstrate the need for this level of placement.

Consumer will have the ability to complete activities of daily living with a minimum number of prompts.

Consumer must be capable of participating in a community day program and learning to use public transportation.

Consumer will be stabilized on medication.

Consumer will not be consistently psychotic.
