Agency History

  • Golden Circle Behavioral Health is a limited liability corporation that was founded in October 1994.

  • We are a “vertically integrated network” of providers including Eyerly-Ball Community Mental Health Services, Goodwill Industries of Central Iowa, and Westminster House, Inc.

  • Golden Circle Behavioral Health secured its first service provider contract in 1997 with the Lead Agency Capitation Pilot Project.  Subsequent contracts have been established for Assertive Community Treatment and Targeted Case Management.

Services Provided

Integrated Services Project:  

This project is a test or demonstration of single stream funding based on a per member yearly rate.  The project provides community based services similar to case management by linking members to available resources utilizing the principles of psychosocial rehabilitation.

Targeted Case Management:

TCM is an individually focused process through which consumers are assisted in building a foundation for personal growth.  Through identifying, securing, and sustaining resources consumers are able to live in a productive and interdependent way within the community.  The major thrusts of TCM are to ensure that consumers have proper evaluations and assessment, linkage to community resources, and coordination and monitoring of service effectiveness.  TCM serves adults with serious and persistent mental illness, mental retardation, and developmental disabilities and children with mental retardation.

Program For Assertive Community Treatment:

PACT-- "a hospital without walls"-- refers to comprehensive outpatient services delivered in the community setting. Services are directed toward the rehabilitation of behavioral/social/emotional deficits and/or amelioration of symptoms of a mental disorder.  Consumers of PACT are primarily patients with severe and persistent mental disorders, and/or complex symptomatology.  Mental health and support services are provided by a multidisciplinary team (including a board-certified psychiatrist, a registered nurse, a licensed mental health professional, a vocational specialist, and other competencies) so consumers can remain in the community.

Service Coordination:

Service Coordination is an individually centered service focusing on consumer strengths, needs, and abilities.  Service Coordination works with individuals to assess needs, set goals, refer to various services, and advocate for consumer needs and rights.  The individual must have a need for the service.  Examples of services funded include vocational services, residential placement, representative payee services, and ongoing general assistance.

Service Coordination serves individuals who are at least eighteen years old and have a primary documented diagnosis of mental retardation, developmental disability, or mental health issues. Most consumers in this program have only one service and require minimal assistance from the Service Coordinator in order to maintain that service.  This may include a few consumers who have Medicaid but do not need Targeted Case Management.  However, the majority of consumers in this program do not have any financial or insurance benefits.  Individuals who are in the process of applying for disability benefits may receive monthly financial assistance until the determination process is completed.  Polk County funds both this program and the services needed by consumers.
